What is an Ocean Form?
An Ocean Form is a digital tool for collecting and sharing patient health information (PHI) safely and securely among patients, providers and systems. Below their patient-friendly interface is a robust, cloud-based security architecture purpose-built to protect privacy while making critical data readily available to those who need it.
How are Ocean Forms used?
Clinics and care teams across Canada use a variety of Ocean Platform tools to send and receive thousands of Ocean Forms every day.

Clinical Patient Forms
These forms are designed to collect specific clinical details from the patient and inform treatment decisions. Clinical Forms can be completed by patients before, during or after their visit, and the information they provide will automatically update their chart.
Administrative forms are designed to improve clinic efficiency and enhance the patient experience by making it easy for patients to contribute directly to their record. This includes updating demographics, completing intake forms or providing consent.
Ocean’s family of eRequest forms facilitate the transfer and back/forth coordination of care among multiple clinicians, clinics and health systems. These forms include those for sending referrals, conducting specialist consults and other submissions to authorities.
