About the Session
This Ocean Horizons session will bring together program and executive leaders from Atlantic provincial partners for a focused discussion on cross-provincial referrals and opportunities to improve how patients access care across provincial boundaries. Provincial Idea Exchanges are an opportunity for provincial teams working with Ocean to collaborate, share experiences and best practices, and explore common opportunities. In this session, participants will discuss:
- How cross-provincial referrals are managed today across the Atlantic provinces.
- Where patients, providers, and provincial programs encounter barriers or gaps in the referral journey.
- Opportunities to improve coordination, visibility, and access to care across provincial boundaries.
- How Ocean eReferral could serve as an enabler for more connected cross-provincial referral pathways and a better experience for patients and providers.
This will be an important regional and jurisdictional discussion, bringing both program and executive perspectives together to explore what may be possible across Atlantic Canada.
